The Skin Club is a separate legal entity from Haven Beauty Collective. The two separate entities occupy the same shared workspace. I understand The Skin Club is entirely separate from any other business operating out of the shared space. I also understand and consent that my Protected Health Information (PHI) will be evaluated in a shared space thus allowing for the potential of disclosure to other operators/clients and third parties within the shared space. I give my full consent to this practice model and agree to The Skin Club's model of practice. I release and hold harmless The Skin Club for any disclosures.
